101 Form Employee formal printed-card

The purpose of the next exercise was to select an existing interface, to research and formulate a problem which can be solved by redesigning the product. I chose “Form 101”- employee's card. This is a Tax Authority form that every employee is required to fill out at the beginning of a new job. This form is intended for tax, social security and health insurance purposes and an official document from the state authorities.

When I analyzed the form I found it consisted of questions and answers which is the principle that guided me in the redesign: The white page at the bottom, is a neutral question page, and the colored form functions as an answer sheet, which is placed respectively above the question form and is submitted to the authorities once completed. The color changes according to the language of the form (Hebrew, Arabic and English).
